Medical records, birth certificates and 3rd party Release of Information
Your records are important to you. We want to make it as simple as possible to get them, but we’re required by law to obtain written information.
Please follow the steps below depending on your request.
To request a copy of your birth certificate
Birth certificates are not issued by the hospital, but by the state government:
To have copies of your Legacy medical or billing records mailed to you personally

For other persons (3rd parties) requesting copies of medical records
If you are asking for the records of another person, you must include proof you are legally allowed to sign for and make healthcare decisions for the patient.
