Legacy expands primary care services in East County
(Gresham, OR) — Legacy Health is expanding primary care services for residents in Gresham and the surrounding East County communities. Legacy Medical Group—Gresham is now offering adult primary care services. The clinic is located on the Legacy Mount Hood campus at 24900 S.E. Stark in Gresham.
The new clinic addresses the need for greater access to quality primary care services in the East County region. “Access to quality primary care services is a foundation of personal health,” said Gretchen Nichols, chief administrative officer for Legacy Mount Hood. “Our commitment as a regional healthcare system is to provide our community with expert medical care they need close to home.”
Internal medicine physicians Robert Bailey, M.D. and Jon Hobson, M.D. have joined the Legacy Medical Group—Gresham practice. Both physicians are board certified in internal medicine and committed to providing personalized and compassionate care. Dr. Bailey emphasizes helping his patient know more about their health through health screenings and reducing risk factors. He also has a special interest in care for the elderly. Jon Hobson, M.D. emphasizes prevention, early diagnosis of illness and treatment to help maintain a high quality of life.
The LMG —Gresham clinic will offer a range of primary and specialty services. The clinic will provide routine healthcare services including annual wellness visits, health screenings, women's health screenings (pap smears, mammogram referrals, bone density testing), wellness evaluations and specialist referrals. The clinic’s highly-skilled team will also address specialized health concerns including immunizations, joint injections, skin procedures, nebulizer treatments, and treatment for chronic conditions such as diabetes, hypertension, asthma, depression, arthritis, and heart disease.
Legacy Health’s primary care clinics are managed as medical homes where the practice is focused on the health of the whole person in all stages of life, for acute and chronic care as well as preventive care. “The expansion of primary care services provides East County families with direct access to Legacy’s expertise and specialized care in a medical home model that provides comprehensive, quality and proactive care,” said Melinda Muller, M.D., FACP, clinical vice president, Primary Care, Legacy Health.
The medical home model provides a coordinated approach focused on the health of the whole person. A medical home offers individually tailored services in a compassionate partnership with physicians and other health providers. Whether patients receive services only at their primary care clinic or from a specialty clinic, hospital or other source, the health home team will coordinate their care for the best possible outcome.
Legacy Medical Group—Gresham is welcoming new patients and accepts many different health plans. The clinic offers patients timely same day and next appointments.
