Connecting and sharing learnings across Legacy Health Partners
By Albert Chaffin, MD
Medical Director, Legacy Health Partners
In LHP’s January newsletter I mentioned our focus this year for supporting provider partners in achieving their goals and delivering the highest quality of care to our patients. We continue strengthening our partnerships with our provider members and their clinic staff. I recently had the opportunity to visit the clinics who won an LHP quality award to recognize and celebrate with their staff and learn more about what made the projects successful.

I was impressed by the advice that teams across adult and pediatric primary care, as well as adult specialty clinics, shared to make a quality project successful. One common element was to recognize that it is the staff who do the heavy lifting and the providers who need to be supporting leaders. I hope to share more tips and lessons learned soon, so we can improve collaboration and outcomes across the entire LHP network.
Consulting services for LHP medical practices
We understand the challenges and opportunities facing healthcare providers today, and LHP is here to support you every step of the way. One of the key benefits of being a physician partner is access to our Quality & Performance Improvement team. Our team members are experienced in PCPCH (Patient-Centered Primary Care Home) medical home attestation, creating and implementing quality improvement projects, supplemental data submission, and workflow consultations. We offer complimentary consultations to help you navigate these processes and optimize your practice for success.
Clinical Guidelines
We are also committed to making our collection of recently updated clinical guidelines more readily available to providers for ease of access. We understand the importance of evidence-based medicine and want to ensure that our providers have the tools and resources they need to deliver the best possible care to their patients.
Looking Ahead
As we move through 2024, we are excited about the opportunities ahead and committed to supporting our physician and advanced practice provider (APP) partners in achieving their goals. Together, we can continue to innovate, collaborate, and deliver exceptional care to our patients. Thank you for your partnership and dedication to improving healthcare in our community.
