New LHP Incentive Distribution Model

We are thrilled to share that the LHP Board approved a new clinical integration incentive model earlier this month. This new model was recommended by the LHP Finance & Contracting Committee and features several significant enhancements, including getting more money in the hands of network practices and providers and reducing the amount of unearned funds each year, while still ensuring that we maintain an adequate reserve fund. This new model will apply to the incentive payouts for the 2024 performance year, which will be distributed in Fall 2025.

Last year, we made the decision to update the distribution methodology, which had not had significant changes made to it for several years. Several possible models were evaluated and LHP staff and leadership worked with our analytics team to refine the details. We appreciate the Finance and Contracting Committee members’ thoughtful review of the new model as well as input from the Quality & Membership Committee before the final approval from the LHP Board. This collaborative effort is one of the most valuable attributes of a provider-led network!

In the coming weeks, your Provider Outreach and Engagement Adviser will meet with you to review the new model. Some key changes you’ll notice:

  • CI Program funds and Shared Savings distribution are calculated separately.
    • CI Program Funds distribution earned by each provider will be a sum of quality, practice contract participation, and citizenships measures.
    • Shared Savings will be distributed based on a provider’s quality score.
  • Practices will receive points for applicable contracts they participate in.
  • Exceptional performance will be rewarded!
    • Previously, providers were rewarded only if they met the measure, now the higher their percentile, the more they earn.

The elements you’ll see carry over from the current model include:

  • Practices and providers will still earn incentive for citizenship measures and attribution tiers.
  • Incentive eligibility is still contingent on completion of the annual CI Education video.
  • Care coordination/PMPM funds will still be distributed to PCPs quarterly.
  • Hospital pool funds are distributed in accordance with achievement of hospital measures.
  • Practices are only eligible for PMPM and Shared Savings Funds from contracts they participate in.

As mentioned above, this new approach to distribution will apply to the 2024 performance year, which will be paid out in Fall 2025. We’re excited to see these enhancements in action as they support the tremendous growth of the value-based contracts and network that has occurred over the last 10 years.

For example, LHP recently compiled a colorectal cancer screening toolkit to help providers succeed in this measure and improve the health of their patients. The toolkit covers how to address common barriers to screening, workflow suggestions for optimizing patient reminders and scheduling, and common billing codes accepted to ensure your coding practices satisfy the measure.

 



Specialty Spotlight: General pediatrics

National Immunization Awareness Month (NIAM) is observed every August to highlight the importance of vaccinations for all ages. Announcements, deadlines, and reminders

  • LHP has been sending reminder emails directly to providers who still need to complete their annual Clinical Integration Education video by September 30. If you haven’t already, go to the LHP video library to complete this requirement and choose one learning module by December 31. Completing both activities helps providers participate in our network and stay informed about resources and relevant topics as you continue your learning.

  • For primary care: Be sure to review the Member Attribution report in Power BI for a list of the LHP patients attributed to you. If you believe a member is attributed to you in error, download the Reattribution Request Form from the LHP Team Site and include the patient’s correct PCP and email the form to LegacyHealthPartners@lhs.org before Sept. 30.

  • Legacy Health Partners (LHP) is accepting applications for our annual LHP Quality Awards that document an improvement project you have been working on. We want to recognize your entire staff for your hard work and help share information and ideas across our entire network. Download the application form and submit it by October 31. LHP provides financial awards to the top three winners ($2000, $1500, $1000 respectively) to help support your efforts and recognizes clinics by delivering team treats, awards, and sharing how practices implemented improvements so other practices can try similar strategies.   • Save the date: Wednesday, October 2 from 5:30 – 7:30 p.m. in the Lorenzen Conference Center at Legacy Emanuel Medical Center. LHP will host our third Clinical Collaboration event of the year, “It’s Getting Hot in Here: Navigating the Latest Strategies for Managing Menopause.” The session will address the physical, emotional, and mental health challenges women face during this life stage and offer evidence-based approaches to care.
