Medication Management Services at Legacy Health

Legacy Medication Management Services has been serving the community for over 30 years, starting as a focused anticoagulation service, it has since expanded to include a wide range of services to improve medication use and access for our patients. Our clinical pharmacists work closely with you, your provider and your pharmacy to help you with your complex and important medications. They can help manage medication regimens for:

  • Anticoagulation
  • Heart Failure
  • High blood pressure
  • High cholesterol
  • Hepatitis C
  • Long-term opioid use
  • Weight Loss

In addition, this service can help patients who take multiple medications and have concerns about managing them or affording their medication.

We also work closely with our Specialty Pharmacy team to provide extra support and counseling for patients using specialty medications which are often high-cost, complex, and challenging to obtain and use correctly.

Whatever your medication support needs are, from managing side effects to feeling confident in how you are using your medications to helping you afford your medications, the Medication Management Clinic is here to help!

Referrals

Legacy Medication Management Services require a referral from your doctor, who must have Legacy Medical Staff affiliation. Your doctor should provide:

  • A completed Legacy referral form
  • Current medication list
  • Current medical problem list
  • Relevant medical chart notes and labs.

Referrals can be made through Legacy’s Epic system (REF111 for warfarin/heparin management, all other services through REF189)
