Journey of Care
Your provider referred you: now what?

Step 1
Getting Started
- Our scheduler will contact you to schedule and send you paperwork.
- You will fill out the paperwork to bring to your first visit.

Step 2
Introductions
- You will meet with your care team.
- They will learn to understand how you’re feeling and make a plan with you.
- This may involve different specialists from our program.

Step 3
Unique Care Plan
- Learn about your pain.
- Try activities to help your body move better.
- Develop skills to work with your pain.
- Attend classes and support groups.
- Try medications, shots, acupuncture or other treatments.

Step 4
Progress
- You may start to notice pain does not bother you as much, and there’s room to enjoy life.
- Life may look different, but you can forge a fulfilling path.
- We are here to help you.
- Usual length of care is 1-2 years.

Step 5
Graduation
- Though you might still have some pain, you now have tools and skills to work with it and live more freely.
- This is when our time together ends.
- We send you back to your primary care doctor for ongoing care.

Step 6
Long-term Plan
- You and your regular doctor will work together to treat your pain in the long term.
- You may come back to see us if you and your provider need more help later on.
