Standard Referral

• We welcome referrals to help address your patient’s pain management needs.
• We do not take over longitudinal prescribing of stable medication regimens.
• Referring providers or PCPs assume longitudinal prescribing upon graduation.
• Our interdisciplinary team partners with you and your patient to improve function and reduce the negative impacts of pain.
• Patients learn how to work with their pain more effectively through medical, movement, physiological quieting and adaptation strategies.
• Average length of care: 1-2 years.

• Epidural steroid injections
• Nerve blocks
• Joint injections
• Trigger point injections
• Medial branch blocks/radiofrequency ablation
• Botox
• Spinal cord stimulators
• Intrathecal pumps
• Other procedures
• Does not include medication management or interdisciplinary care
         

Consult Only

• Meet for 1-2 visits with a medical provider for comprehensive treatment recommendations.
