Legacy’s member engagement program makes big strides
As Legacy Health works to grow covered lives to 300,000 people by 2025, it has become clear that we need a proactive onboarding and engagement strategy. Legacy will become a top performer in patient access and experience through Population Health’s new member engagement program. This program is designed to work with our health plan partners to deliver a cohesive and seamless onboarding process and provide access to Legacy Health Partners (LHP) primary care clinics, including Legacy Medical Group as well as independent practices.
When a patient changes insurance through work or for other reasons, they often face a difficult time coordinating any ongoing care and figuring out which provider they can see on their new plan. The member engagement program will work with PacificSource and other health plans to reach out to patients and offer them a seamless transition to their new plan and help connect them with a new provider in LHP if needed. This work will support our efforts to co-manage the quality, cost, and experience of their care.
Beginning in August, the member engagement program will work with members from certain PacificSource and Regence plans and will expand to other managed care populations soon. The member engagement team will facilitate access across LMG and LHP primary care practices, which will eliminate variation in scheduling practices and customize the member experience. Additionally, the team will look for opportunities to integrate access to Legacy’s care management program and expand capabilities and services over time.
Anne Percival, who is currently out on leave, will lead this work as the member engagement manager. Anne brings a wealth of knowledge to the position having worked previously at Legacy as the interim member engagement manager, Population Health consultant with the Legacy Population Health team, and field operations adviser when she joined Legacy in 2016.
