The Gender Care Services Program at Legacy Health creates a safe, affirming and welcoming environment for its specialty care and services. Our compassionate and deeply experienced staff provide:

How to make an appointment

To request or begin the process of becoming a new patient, please fill out this online form.  There will be a place on the form to let us know if you'd like to speak to someone over the phone.  

If you are having trouble filling out the form, you can call us at 503-413-5915 during business hours.  We are open from Monday to Thursday, 8 a.m. to 4:30 p.m., and Friday, 8 a.m. to 4 p.m.

Legacy Health supports the LGBTQ2IA+ community, especially our gender-diverse friends and neighbors. Legacy Health adheres to the Standards of Care 8 as published by the World Professional Association of Transgender Health.

The Legacy Gender Care Services Program includes patient navigators as part of our team. They are available to provide personalized support, answer questions, and help you navigate your healthcare journey.

They can help you:

  • Explore available care options.
  • Learn about our many health partners.
  • Discuss transition-related questions.
  • Find community resources.
  • Answer insurance questions.
